MEMBERS

Members can log into their accounts by pressing the members button. You will need your account
number, user name, and password to access your account.

mymiice EMERGENCY

add access number
You can add multiple access numbers to your account. When you add an access number
you will also need the product key code for that access number in order to activate it.

edit
You can edit the access numbers associated with your account. When you press edit,
you can add a description and change the status. Remember to save your changes when
you are done.

description
access number list. In example, you can add a description of “wallet card” for your access
number associated with your wallet card.

status
You can change the status of an access number to active or inactive. This may be necessary if you ever have to replace either the wallet card or the wristband.

View

This provides a quick view of your emergency access report. Your emergency access report
contains all of the information which you have selected to share by “allowing emergency access”.
This is also the report that EMS and other first responders see. Keep in mind that this report
contains all of the information for internet access viewing only. The information available through our
phone service will not include all of the information available through the internet. We recommend
that you call the toll free number to verify your information accessed through our phone service.

account information

This provides access to your account information. You can view your account number, user name,
active date, and expiration date. You can view and change your password, email address, and
billing information. Remember to save your changes when you are done.

personal information - name

add former name
When your account was set up, you may have elected to enter your current name.
Through your name list, you can also add a former name. This may be beneficial for
someone who wishes to include prior names which may be associated with marriage,
divorce, adoption, or other reasons. If you include a nickname, the nickname will replace
the first name on all reports and through the phone service. For example, if your true name
is George, but every knows you as Chuck, then Chuck will be displayed on all reports.
Remember to save your changes when you are done.

personal information - address

add new address
When your account was set up, you may have elected to enter your current address. Your
current address should be your primary residence. Through your address list, you can also
add a new address. You can use this feature to change your address on a temporary
basis. You may want to do this if you are vacationing and know the address of where you
will be staying. You may have a second home which you stay at for long periods of time,
or you may be traveling for business and want to include the hotels where you will be
staying.

personal information - identification

You can add personal information like gender, race, height, weight, eye color, hair color,
blood type, and primary language spoken. It is recommended that you add your primary
language spoken. This is critical and can save a lot of time if the EMS or other first
responders knew which language translation is required. This is especially important if your
primary language spoken is not English, or you travel outside of the country where you are
traveling does not use English as a primary language.

personal information - birth

You can keep a record of your birth information. You can add gender, date of birth, time of
birth, weight, length, name, and place.

medical information - medical condition

add medical condition
You can add any medical condition you may have. If your medical condition does not
appear on the list, please contact us at support@mymiice.com so we can add it. You may
want to consult your doctor to ask which medical conditions are important to share in case
of an emergency. In the edit screen, you can change the status, description, start date,
end date, last episode date, and any other comments you wish to share.

medical information - allergies

add allergy
You can add any allergy you may have. If your allergy does not appear on the list, please
contact us at support@mymiice.com so we can add it. You may want to consult your
doctor to ask which allergy is important to share in case of an emergency. In the edit
screen, you can change the severity, description, start date, end date, last reaction date,
and any other comments you wish to share.

medical information - medications

add medication
You can add any medication you take including prescription, over the counter,
supplemental herbs, or vitamins. If your medication does not appear on the list, please
contact us at support@mymiice.com so we can add it. You may want to consult your
doctor to ask which medications are important to share in case of an emergency; and
which medications you would feel comfortable sharing publicly. You may not want to share
medications which you feel are private like antidepressants, medications tied to conditions
which you don’t want to share, or any other medication for any other reason. However, we
do recommend that you consult your doctor regarding all of your medications so that you
can make an informed decision regarding which medications you share and the medical
implications regarding the ones you don’t. In the edit screen, you can change the
medication names, prescribing physicians, and pharmacy numbers, fill quantity, strength,
start date, end date, directions, instructions, form, dose, intervals, frequency, and any
limits.

strength
The strength of the medication, for example, “30 mg”.

dose quantity
The dose quantity of the medication, for example, “take two tablets”.

frequency
The frequency of the medication, for example, “two times”.

interval
The interval of the medication, for example, “every four hours”.

medical information - immunizations

add immunization
You can add any immunization you have had. If the immunization does not appear on the
list, please contact us at support@mymiice.com so we can add it. In the edit screen, you
can change the status, name, administrating physician, date given, route, site, and name
and lot number of the manufacturer.

emergency contact information

add emergency contact
You can add multiple emergency contacts. We recommend that include relatives which do
not live in your immediate area. Include as much information as you can. Be sure to
include the relation of the emergency contact, for example, your mother, father, uncle,
brother-in-law, step daughter, etc. Keep in mind that the emergency contacts which you
allow emergency access, the EMS or other first responders will have to listen to all of the
information and it may be too long. For this reason, you will want to limit the shared
emergency contacts to those emergency contacts which are most relevant.

You can add all of your physician information including, specialty, address, phone
numbers, fax number, and email address.

add pharmacy
You can add all of your pharmacy information including, pharmacy name, address, phone
number, fax number, email address and website. You can include all types whether it’s the
local pharmacy, mail order pharmacy, or others.

health insurance information

add insurance
You can add all of your health insurance information including, primary, secondary, ID
numbers, name, address, phone number, fax number, email address, payment method, co-
pay, start date, end date, and termination date.
